Key Features of Matrix Forms
- Matrix question types include Yes/No, Single Answer, and Numeric questions.
- When creating or editing a form, web admins can add up to three Matrix questions per form.
- Users can configure up to 10 columns and 25 row items in a Matrix question.
- Offline mode is fully functional—responses will sync automatically once the Rep regains internet access.
- Once a Matrix question is added to a form, its question type cannot be modified (though it can be deleted if necessary).
How to Create a Form
Follow these steps to create a form with Matrix questions:
1. Click on the New Form button.
2. Enter the required details for a new Form. Add questions to your form and select Matrix as the question type.
Matrix questions function using a grid-based layout, where:
- Rows within a Matrix represent different items, options, or categories for which Reps must evaluate and provide answers.
- Columns within a Matrix capture the questions related to the items listed in the rows. At the time of this document being published, supported Matrix question types include: Yes/No, Single Answer, and Numeric.
3. After you create the Matrix question type, it will look like shown below.
4. From this sidebar, you can seamlessly view all your questions, which is handy for Forms with several options and rearranging a newly added Matrix question.
5. Once your form is complete, click Save to finalize it.
